REQUEST FOR ISSUANCE OF A DUPLICATE CERTIFICATE OF ORIGIN (C OF O) FOR A TOYOTA MOTOR VEHICLE

A duplicate of the original Certificate of Origin document is required for the following reason:

Not Received
Lost By FMC Selling Dealer Delivery Dealer
Original C of O mutilated
Others

Within 60 days of report of sale (Fleet auto RDR)

If the original MSO or its remains can not be returned to TMS Corporate Fleet, a written statement from the delivery
dealer indicating the applicable vehicle has not been titled must be provided (if the selling dealer and delivery point
are in different states).

After 60 days of report of sale (Fleet auto RDR)

If the original MSO or its remains cannot be returned to TMS Corporate Fleet, a written statement (title search) from
the state agency responsible for the titling of motor vehicles must be provided by the original wholesale dealer verifying that the subject vehicle has never been titled in the state where loss of the original C of O may have occurred. A duplicate C of O will not be issued until such documentation is submitted to TMS Corporate Fleet.

I certify that the above-described vehicle is new and has never been titled in any state. In the event that the original C of O is found I agree to return it to Toyota Motor Sales, Inc. I agree to indemnify Toyota Motor Sales , Inc. and hold it harmless from any suit, action, proceeding or claim of any kind brought or initiated as the result of the issuance of the duplicate Certificate of Origin.
